Report an Incident

Incidents occurring on or involving Ohio State property or while participating in a university activity should be submitted on the university’s reporting portal: RISKONNECT

What You Need: Ohio State email (name.#) and password

Confirmation of Input: Upon submission, Riskonnect will confirm input was received successfully.

Who will contact you?  A representative from the Office of Enterprise Risk Management.

SPECIAL NOTE:  If the incident is an emergency and needs immediate attention, please contact Public Safety at 911 or 614-292-2121.
