Certificate of Insurance
A certificate of insurance is a document that demonstrates evidence of insurance policies in effect at the time of issuance. It is not an insurance policy, does not convey any rights to the certificate holder, and is often required to meet contractual obligations.
Evidence of insurance for the following lines of coverage can be found below:

- Student Practicum Coverage
If you require a contract specific certificate of insurance, please submit your request to insurance@osu.edu. A copy of the contract must be included for consideration.

Claims History Request
Professional Liability Information Requests
Requests for verification of professional liability coverage for former Nursing and Veterinary students should be submitted to insurance@osu.edu. A copy of the information consent and release form executed by the former student must be included for consideration.
OSU Wexner Medical Center Provider Information Requests
Requests for verification of professional liability coverage for OSU Wexner Medical Center providers and former College of Medicine students should be submitted to insurance.usip@osumc.edu. If a request is submitted by a third party, the clinician must provide a signed release.
