Benefits Confirmation Statements now available online

Faculty and staff eligible for Ohio State benefits can review their 2020 Benefits Confirmation Statement in Employee Self Service through December 3, 2019. Employees will also receive a copy of the 2020 Benefits Confirmation Statement at their home addresses. This is the final opportunity, outside of a qualifying event, to change 2020 benefits elections.

Changes can be made by writing corrections directly on the statement and adding a signature and date. Corrections can be submitted to HR Customer Service by mail (1590 North High St., suite 300), fax (614-292-7813), in-person drop-off (1590 North High St., suite 300), or email.
